An obesity surgery mortality risk score (OS-MRS) is a numeric scoring system used to assess the risk of mortality associated with bariatric and metabolic surgery for patients with severe obesity. It was developed by industry experts in collaboration with academic, governmental and clinical partners as part of an international initiative to improve patient safety in this field. OS-MRS scores are based on 11 different variables which can be ranked from 1 (lowest) to 10 (highest), including aspects such as age, weight, gender, severity of obesity and type of procedure performed. The higher the score the greater risk that an individual will experience complications during or after their procedure.

The most commonly used OS-MRS tool is called 'Obesity Surgery Mortality Risk Score' or 'OSSAT'. This scoring system was first proposed in 2007 by researchers at University College London and has since been validated in various studies across multiple countries worldwide. In general, scores using OSSAT range from 0–50; however depending on the population studied there can be differences between low (20). Patients who receive a score ? 20 have showed significantly higher risks than those who have a lower one < 10>
