Answer: The patient is intubated with a cuffed endotracheal tube which is advanced into the trachea. The lungs are then deflated by clamping the bronchi and the chest is opened. The surgeon inserts retractors to hold the rib cage open and proceeds to excise the diseased portion of lung. After the surgery is complete, natural ventilation is restored by removing the clamp from the bronchi and slowly withdrawing the endotracheal tube while watching for chest rise. If there has been any injury to the lung parenchyma, gentle positive pressure ventilation will be applied until it is repaired.
