The Selenium test is a blood test that measures the level of selenium, an essential trace mineral that the body cannot produce on its own and must obtain entirely from dietary sources including Brazil nuts, seafood, meat, eggs, and certain grains. Selenium functions as a critical component of selenoproteins, a family of proteins with diverse and essential roles in antioxidant defence, thyroid hormone metabolism, immune function, DNA synthesis, and reproductive health. The most important selenoproteins include glutathione peroxidase, which protects cells from oxidative damage, and iodothyronine deiodinase, which converts the inactive thyroid hormone T4 into the biologically active T3. Selenium deficiency, while less widely recognised than iron or Vitamin D deficiency in India, is an increasingly documented nutritional problem particularly in regions with selenium-depleted soils, among vegetarians with limited dietary diversity, and in patients with chronic gastrointestinal malabsorption conditions. Deficiency is associated with thyroid dysfunction, impaired immune response, increased susceptibility to viral infections, male infertility, and in severe cases Keshan disease, a cardiomyopathy caused by extreme selenium depletion first described in selenium-deficient regions of China but relevant wherever soil selenium is low. Conversely, excessive selenium intake from supplements causes selenosis, a toxicity syndrome with characteristic symptoms. The test involves a simple blood draw completed in under five minutes.

Assessing selenium status in patients with HIV, cancer, and chronic inflammatory conditions where selenium depletion is common, impairs immune function, and is associated with poorer clinical outcomes.

The Selenium test measures the concentration of selenium in the blood, reported in micrograms per litre (mcg/L) or nanomoles per litre (nmol/L). Normal Selenium Range The standard reference ranges used across most Indian diagnostic laboratories are as follows. For adults, a normal serum selenium level is between 70 and 150 mcg/L. Optimal selenium levels for maximum selenoprotein activity and health benefit are considered to be between 80 and 120 mcg/L by most nutritional medicine specialists. Interpreting Selenium Results A selenium level below 70 mcg/L indicates deficiency and warrants dietary assessment and selenium supplementation under medical supervision. Levels below 40 mcg/L represent significant deficiency associated with impaired antioxidant defence, thyroid dysfunction, immune suppression, and in severe cases cardiomyopathy. A selenium level between 70 and 80 mcg/L represents a borderline low range where subclinical deficiency effects on thyroid function and immunity may already be present, particularly in symptomatic patients or those with known risk factors for deficiency. A selenium level above 400 mcg/L raises concern for selenium toxicity or selenosis, which can cause hair loss, brittle nails, peripheral neuropathy, gastrointestinal disturbance, and in severe cases respiratory distress and cardiac complications. Selenosis is almost exclusively caused by excessive supplementation rather than dietary intake.

Selenium deficiency produces non-specific symptoms such as fatigue, hair loss, and recurrent infections that overlap with many common conditions, making it easy to overlook without targeted testing. It is also rarely included in standard nutritional screening panels, and awareness of soil selenium depletion as a regional dietary risk factor remains limited among both patients and general practitioners in India.

Selenium is required by iodothyronine deiodinase enzymes that convert the inactive thyroid hormone T4 into the biologically active T3, so deficiency reduces T3 availability and contributes to hypothyroid symptoms even when iodine intake is adequate.
