The Homocysteine test is a blood test that measures the level of homocysteine, an amino acid produced as a byproduct of metabolising methionine, a protein found in meat, eggs, and dairy. Under normal conditions, homocysteine is converted into harmless substances with the help of vitamin B6, B12, and folate. When these vitamins are deficient or certain genetic enzyme defects are present, homocysteine accumulates in the blood, a condition called hyperhomocysteinaemia.
Elevated homocysteine is an independent risk factor for cardiovascular disease including heart attack and stroke. It damages blood vessel walls, promotes abnormal clotting, and accelerates atherosclerosis. Normal Homocysteine Range in India
A homocysteine level below 15 umol/L is considered within the acceptable range for adults. Many cardiologists recommend an optimal target below 10 umol/L for patients with existing heart disease or multiple risk factors.
Interpreting Homocysteine Results
A level between 15 and 30 umol/L indicates mild hyperhomocysteinaemia, most commonly caused by B12, folate, or B6 deficiency. This is the most correctable and frequently encountered elevation in clinical practice.
A level between 30 and 100 umol/L indicates moderate hyperhomocysteinaemia, where nutritional deficiency combined with possible enzyme defects or renal impairment is investigated. Cardiovascular and thrombotic risk is significantly elevated at this range.
A level above 100 umol/L indicates severe hyperhomocysteinaemia, most commonly seen in homocystinuria, a rare inherited metabolic disorder, and requires urgent specialist evaluation.
