The Alanine Transaminase (ALT) test, commonly known as the SGPT test, is a blood test that measures the level of the enzyme alanine transaminase in the blood. ALT is an enzyme found predominantly in liver cells and plays a key role in converting proteins into energy for liver cells. Under normal conditions, ALT is present in the blood in small amounts. When liver cells are damaged or inflamed, they release larger quantities of ALT into the bloodstream, causing blood levels to rise significantly. Because ALT is found almost exclusively in the liver, it is considered the most specific blood marker of liver cell damage currently available in routine diagnostics. An elevated SGPT is one of the earliest and most reliable signs that the liver is under stress, making this test an essential component of any liver function evaluation. It is widely used across Indian hospitals and diagnostic centres as part of the Liver Function Test (LFT) panel and as a standalone test when liver disease is suspected. The test involves a simple blood draw of approximately 2 ml from a vein in the arm and is completed in under five minutes.

The SGPT test measures the concentration of Alanine Transaminase enzyme in the blood, reported in units per litre (U/L). Normal SGPT Range in India The normal reference range for SGPT in adults as followed by most Indian diagnostic laboratories is as follows. For men, a normal SGPT level is between 7 and 56 U/L. For women, a normal SGPT level is between 7 and 45 U/L. Values may vary slightly between laboratories depending on the assay method used, which is why your report will always include the specific reference range of the processing lab. Interpreting SGPT Results A mildly elevated SGPT between 56 and 150 U/L may indicate early fatty liver disease, mild hepatitis, or the effect of certain medications. This range is increasingly common in Indian patients with metabolic syndrome, obesity, or type 2 diabetes. A moderately elevated SGPT between 150 and 500 U/L suggests significant liver inflammation or injury and warrants further investigation including a full liver function panel, ultrasound, and viral hepatitis screening. A severely elevated SGPT above 500 U/L indicates acute liver injury and may be seen in acute viral hepatitis, drug-induced liver toxicity, or ischaemic hepatitis. This requires urgent medical evaluation. It is important to note that a single elevated SGPT reading does not confirm a specific diagnosis.
